Systematic Error Reporting
As a general principal, try to be consistent and informative.
i.e. when you report an issue, please follow the "W's";
- Who (name /user ID)
- When (date and time)
- Where (office)
- Which PC (asset ID)
- What you were doing (data entered)
- Why it appears to be wrong
- How this affects you (and what the impact is)

Their names are What and Why and When
And How and Where and Who.
